Affiliate Enrollment

Step-by-step process for admins to enroll a new affiliate.

Collect Affiliate Information

Before starting, request the following from the affiliate:

  • Company Name
  • DBA (if applicable)
  • Company Address
  • Contact Name & Title
  • Phone Number
  • Email Address

Required documents for enrollment:

  1. Affiliate Agreement — generated via Contracts and sent for signature
  2. Direct Pay Payee Setup Form – FORTH LLC — sent alongside the affiliate agreement
  3. Tax Form: W-9 (US entities) or W-8BEN-E (international entities)

Do not proceed to Step 1 until all three documents are signed, completed, and returned.

Create the Company

Navigate to Admin Page → Companies → Add Company. Mark the company as Active and complete all required fields:

  • Company Type, Parent Company, Company Name
  • Contact Name, Address, City, State, ZIP Code
  • Phone Number, Email Address, EIN

Save the company before proceeding.

Create a Team in Forth

Navigate to Forth and create a new Team for the affiliate company. Ensure the team name clearly matches the company name. Confirm the team is properly linked and active before proceeding.

Create the Payee

Create a new payee for the affiliate and complete all required fields:

  • Payee Name — keep it short, avoid full company name if long
  • Routing Number, Bank Name
  • Account Type, Account Number, Name on Account

Ensure "Forth Pay Payee" is checked before saving.

Create the Compensation Template

Create a new Compensation Template. Under Available For, select All Companies.

Payee Assignment:

  • Payee 1: Us (Company)
  • Payee 2: Affiliate (the payee just created)
  • Payee 3: Consumer Defense Partners

Fee Setup:

  • Fee 1: Fill out for Payee 1 and Payee 2 using agreed-upon percentages
  • Fee 2 & Fee 3: Assign under Consumer Defense Partners at 100%

Save the compensation template before proceeding.

Assign Comp Template to Enrollment Plans

Navigate to the applicable Enrollment Plans. Add the new compensation template under Comp Templates To Show.

Verify Shared With Companies:

  • If All Companies is selected — no further action needed
  • If not, manually select the new affiliate company

Save changes before proceeding.

Configure Webhooks & Triggers

After assigning the comp template, determine if the company has any recurring fees (including referral fees associated with another company).

If the company HAS recurring fees:

  • Navigate to the Comp Template Green States Webhook — add the new company along with the Comp Template ID
  • Navigate to Triggers — update the Comp Template Green States trigger to include the new company

If the company has NO recurring fees:

  • No changes needed in the webhook
  • Navigate to Triggers — update only the Comp Template Green States – PPDS trigger to include the new company

Verify the correct configuration based on fee structure before closing.

Grant Credit Pull Access

Navigate to Integrations. Enable credit pull access for the new affiliate, if required.

Skip if credit pull is not applicable for this affiliate.

Grant ClixSign Access

Navigate to Settings. Enable ClixSign access for the affiliate.

Create a New User for the Affiliate

Create a new user under the newly created company. Ensure the correct Company is selected. Assign either Affiliate Admin or Aff Admin role, depending on whether capped payment plans are required.

Required User Details:

  • First Name, Last Name
  • Email Address, Phone Number

Additional internal users to create under the new company:

  • Diana
  • Ada
  • Underwriting
  • Banking

Save the user before proceeding.

Update Internal Tracking Spreadsheets

Update the following spreadsheets to complete enrollment:

  • Affiliate Splits Expanded — in the Affiliates spreadsheet
  • Funded Report — add the affiliate from the Closed Deals Report spreadsheet
  • Totals (Data) — in Affiliates Master spreadsheet
